Athletic Department

In order for a Saint Lawrence student to participate on an athletic team, the following forms must be on file with the Athletic Department BEFORE the student joins a team.  Students are not allowed to practice or participate in any contests until all COMPLETED forms are submitted.

1. Medical Clearance Form (2 pages) - Every student-athlete must be medically cleared by a doctor to participate in athletics.  This clearance expires each year on May 31 and must be renewed annually .  

2. Athletic Registration Form (2 pages) - Student-athletes and their parents/guardians must complete an Athletic Registration Form for EVERY sport the student wishes to participate.  Athletic fees must be attached to the registration form.  Families with financial hardships should contact the Athletic Department when submitting their registration form. 

3. Transportation Form (1 page) - Parents must fill out a Transportation Form detailing which modes of transportation are acceptable for their student.  One Transportation Form clears an athlete for an entire school year, expiring on May 31.
